What to Take to a Medication Review

Vinay ChandraBy Vinay ChandraNovember 1, 2026No Comments6 Mins Read

It is surprisingly hard to remember the name of an eye drop when someone asks across a desk. You can picture the bottle in the bathroom cabinet and remember the colour of the cap. The pharmacist needs the words on the label. The evening before a medication review, make a list while the containers are within reach. Ten quiet minutes at home can give you more time at the appointment for a question that really matters to you.

    Walk through an ordinary day

Start where you get up. What do you reach for before breakfast? What do you keep in your bag, in a desk drawer, by the sink or beside your bed? Include the things you use occasionally. A cream for a flare-up, a spray you carry when travelling and an over-the-counter medicine you took last weekend are all easy to forget when the first question is about prescriptions. If you have a caregiver who helps with medicines, their copy of the routine can fill in details you rarely handle yourself.

The US Food and Drug Administration recommends keeping a current list of prescription medicines, over-the-counter medicines, vitamins and supplements. Use a format you will actually bring out in the appointment. A folded sheet in your wallet works. So does a document on a phone if you can open it without hunting for a password. Put the date at the top and leave a little space between entries. A list you can correct in front of the pharmacist will earn its place in your bag.

Copy what the label says, then describe what happens at home

For each item, record its name, strength and form as printed. Add why you understand you take it, when and how you actually use it, and who prescribed or recommended it if you know. You can copy a complicated name letter for letter or photograph the label and ask for help at the appointment. If you cannot read a figure, write “unclear” beside it. A well-placed question mark is more useful to the reviewer than a plausible number you supplied from memory.

Keep the instructions and your routine in separate sentences when they differ. Perhaps the label says “in the evening,” but the container has been sitting unopened since you came home from hospital. Say so, and note roughly when the change happened. That gives the pharmacist a clear question to resolve with you. A review is also a good time to mention practical barriers. A cap you cannot turn or a label printed in tiny type can explain a gap that would otherwise be invisible on a refill list.

Bring the evidence you have for uncertain names

Original packaging, a current prescription list and a recent discharge letter can each contribute a different detail. Ask the clinic before travelling if an item needs refrigeration or is difficult to transport. A clear photo of the front and the dispensing label may help the reviewer identify which information is missing. Check that the photo is in focus while you are still at home. Store it privately because a dispensing label may include your address, an identifier or the prescriber’s details.

If a product came up in an online search, label that information as a search result. For example, hilma biocare shop identifies a retailer in this sponsored feature. Saving a website under “questions to ask” keeps a product you merely researched out of your list of medicines you actually take. If you have used an item and no longer have its packaging, tell the pharmacist what you remember, where your information came from and which details still need to be established.

Make room for your most important question

Bring one or two questions on paper, in the language you find easiest. “My old list and the hospital letter give different instructions. Which document should I be using?” is specific enough to start a conversation. So is “What should I do with the two containers that appear to have the same name?” Keep both entries visible for the reviewer instead of deleting one because it looks like a duplicate. A pharmacist can compare the active ingredient and directions against your records.

A second note can cover how you have felt. Record a symptom in your own words, when you first noticed it, how long it lasted and whether it has happened again. For a past reaction, include the name you remember and what actually happened. “An itchy rash the following morning” gives the clinician better information than a family recollection of “something didn’t agree with me.” You can describe an experience and ask whether a medicine contributed without deciding the cause in advance.

Remember to separate a brand from a specific item. A screenshot showing Hilma Biocare identifies the brand named on the linked retail site. It leaves questions about the exact product, ingredients, strength and what a person used. Bring those details from the actual package when possible. If you saw the name but never used the product, the conversation can begin there. The same approach works for an unfamiliar umbrella brand that appears across several products in a family medicine cabinet.

If several people help you with appointments, agree beforehand about what you want shared and who will bring the latest list. A family member keeping track of appointments might have an older copy than the person who picks up your prescriptions. Putting the versions side by side can uncover a difference worth asking about.

Leave with a record you can use

Before you put the paperwork away, ask the reviewer to go through the decisions with you. Which version of your list should you keep? Who will clarify any outstanding discrepancy? If another clinician needs to make a decision, ask how and when you can expect to hear from that person. Write the details beside the question while you are still in the room. If you wish, a trusted person who comes with you can take notes while you talk.

At home, put the updated list where you will find it before the next appointment. Date it and mark any older copies so you can tell them apart. Give people who help with your medicines the updated information that you want them to have. The next time a clinician asks about those eye drops, you can hand over the exact label details and spend the conversation on what you wanted to ask about them.

Editorial sources include the US Food and Drug Administration’s Create and Keep a Medication List for Your Health, and its patient guide to talking with a pharmacist. Both discuss what to record and what to bring up during a review.
